Inconsistency between media queries and qa_is_mobile_probably()

Slightly hacky since answers/comments don’t have titles. Probably need
to switch to schema.org microdata instead.

Added horizontal lines in Snow theme and increased spacing in
Classic/Candy themes to help distinguish between each plugin and its
options.

Handle button text with double quotes.

Add styles for cancel button on wide form.
